【摘要】目的 以社区抗高血压治疗的前瞻性随访队列为基础，观察北京降压0号片降压效果及不良反应，以及对血脂等生化指标的影响，为科学合理使用降压0号提供科学依据。方法 依托南京市慢性病防治体系，以新诊断高血压患者为研究对象，根据医嘱和患者自身需求，将其分为北京降压0号组和常规治疗组，进行前瞻性队列研究，随访并检测其血压、血液生化指标以及记录不良反应等，队列随访至2012年12月底。
结果 北京降压0号组和常规治疗组分别有390人和392人完成随访，平均治疗时间为2.17年。两组降压有效率分别为94.9%和97.4%，两者无统计学差异（χ2=3.52，P=0.061）；两组不良反应发生人数分别为6人和2人，也无统计学差异（χ2=2.04，P=0.153）；两组总胆固醇、甘油三酯、谷草转氨酶、谷丙转氨酶、血清尿酸和空腹血糖在随访结束时亦无统计学差异（P值均大于0.05）。结论 基于前瞻性队列研究观察，北京降压0号具有和常规治疗药物类似的降压效果和安全性，对血液生化指标，尤其是血尿酸代谢无不良影响。
【 abstract 】 objective to community based on the prospective follow-up cohort of antihypertensive treatment, observation of Beijing step-down 0 antihypertensive effect and adverse reaction, as well as the influence on blood lipid and biochemical indexes such as, provide scientific basis for scientific and reasonable using step-down 0. Method based on chronic disease prevention and control system of nanjing, with a new diagnosis of hypertension patients as the research object, according to the doctor's advice and the patient's own needs, it can be divided into Beijing step-down 0 group and conventional treatment group, prospectie cohort study, follow-up and detect the blood pressure, blood biochemical indices and record adverse reactions, such as queue follow-up until the end of December 2012.
Results Beijing step-down 0 group and routine treatment group, 390 and 392 respectively to complete follow-up treatment time is 2.17 years on average. Two groups of antihypertensive effectiveness were 94.9% and 97.4% respectively, both no statistical differences (chi-square = 3.52, P = 3.52). Two groups of adverse drug reactions occurred number 6 and 2, respectively, also no statistical difference (chi-square = 2.04, P = 2.04). Two groups of total cholesterol, triglyceride, aspertate aminotransferase, alanine aminotransferase, serum uric acid and fasting glucose at the end of the follow-up also no statistical difference (P value were greater than 0.05). Conclusion based on the prospective cohort study was to observe, and regular treatment with the Beijing step-down 0 similar antihypertensive effect and safety of the blood biochemical indexes, especially no adverse effect on blood uric acid metabolism.
